The best Zendesk CSAT and survey app in October 2026 is Nicereply, the only one to score 10 of 10 on our five public criteria, followed by SweetHawk's Survey and Simplesat for Support, which share a second tier on 9. Zendesk's Marketplace files 82 apps under Surveys and Reviews, 81 of them from third parties, and 51 of those have no reviews at all. Many are review-management or product-feedback tools rather than surveys. We scored 13 apps that send CSAT, NPS or Customer Effort Score surveys after a ticket, or put the results in front of agents, and we show every score.

Key takeaways

  • Nicereply scores 10 of 10, the only app on this list with more than 50 reviews and an average above 4.5.
  • SweetHawk's Survey and Simplesat for Support share the second tier at 9; both write responses back to the ticket and publish full price lists.
  • Survey prices are built three ways here: by responses (Nicereply from $59 a month), by account (Survey from $100 a month) or per agent (from $1).
  • Seven apps tie at 7 of 10, mostly because they have fewer than 10 reviews or work from the nav bar rather than the ticket.
  • A survey app that posts ratings as comments or writes fields can reopen a ticket or wake a trigger, so an AI agent's conditions need to account for it.

Tiers: 10 (app 1), 9 (apps 2-3), 8 (app 4), 7 (apps 5-11), 6 (apps 12-13). Averages are computed from the Marketplace catalog's per-star counts. Prices were read on each live listing and vendor page on October 29, 2026.

How we scored them

We started from every app in the Marketplace's Surveys and Reviews category and added apps from other categories whose name or description mentions CSAT, NPS or customer effort surveys. We kept third-party apps whose main job is a post-ticket survey or showing its results in Zendesk Support. Each app is scored 0 to 2 on five criteria.

Works in the ticket scores 2 only for apps placed in the ticket sidebar or editor that also act on the ticket, per each app's locations in the Marketplace catalog and its listing. Read-only sidebars, nav-bar apps and apps whose sidebar only connects an account score 1. Apps with no Marketplace install, where the listing points you to the vendor's site, score 1 on setup. Tier lists with a custom Enterprise tier score 2 on price. Ties are broken by review count, then installs, then the most recent update. Inside a tier, the order isn't a quality ranking.

Zendesk's built-in CSAT, outside the ranking

Zendesk's own CSAT asks one rating question, with optional follow-up questions only after a negative rating, and is sent only on Solved tickets, about 24 hours later by email or immediately in messaging, on Suite Growth and above or Support Professional and above, per Zendesk's documentation. Our guide to how Zendesk CSAT works covers the scales and score formula, and setting up CSAT surveys in Zendesk walks through the switches. The apps below earn their price when you need NPS or effort scores, more than one question, answers in ticket fields, or surveys on channels the built-in survey doesn't cover the way you want.

Top tier, 10 of 10: Nicereply

1. Nicereply: CSAT, NPS and CES with results inside the ticket, 10/10

Nicereply measures satisfaction, effort or loyalty with a survey at the bottom of every email, in macros, or sent by trigger after a ticket or chat is solved. Responses come back into Zendesk as comments and custom field values, and a sidebar widget shows the customer's ratings on the ticket. It has 110 reviews averaging 4.75, the only app here past both thresholds for 2 points on reviews, and was updated in March 2026.

Scores: reviews 2, price 2, works in the ticket 2, setup 2, maintained 2.

The listing says "Free integration" with "Additional fees may apply"; the fees are on Nicereply's pricing page. Plans run from Starter at $79 a month (100 responses, 3 users) to Business at $449 (2,500 responses, 50 users), or $59 to $359 a month billed annually, with a 14-day trial. Every plan has CSAT, CES and NPS and the Zendesk integration; only response and user limits change. Pricing by responses means the bill tracks how many customers answer, not how many agents you have. That business model earns the vendor more as your response rate climbs, so check which tier your current volume lands in. Setup needs a Nicereply account first, and the listing says to connect the two in Nicereply before clicking Install.

Best for: teams that want CSAT, NPS and CES from one vendor with results on the ticket.

Second tier, 9 of 10: Survey and Simplesat for Support

Both lose one point on reviews: each has fewer than 50, so neither reaches the 2-point bar despite near-perfect averages. Review count sets the order.

2. Survey: multi-question surveys stored in ticket fields, 9/10

SweetHawk's Survey builds surveys with any number of questions: good/bad, happy/indifferent/sad, 1-5 stars, 0-10 NPS, single and multiple choice, and free text. It sends them in Zendesk emails or Sunshine Conversations, stores answers in custom ticket fields for Explore reporting, and can start triggers or further surveys from a response. It can also sync with Zendesk's built-in satisfaction rating and skip customers asked in the last X days. It has 49 reviews, all five stars, and was updated in April 2026.

Scores: reviews 1, price 2, works in the ticket 2, setup 2, maintained 2.

The listing shows three flat monthly plans after a 14-day trial: Core at $100, Pro at $180 (adds select and free-text questions, Sunshine Conversations, up to 5,000 tickets a month) and Enterprise at $300 (multi-brand, unlimited tickets). Chris Bardos wrote on its listing, over seven years ago: "It is heads and shoulders better than the standard Zendesk satisfaction rating."

Best for: teams that want survey answers as ticket fields they can report on and build triggers from.

3. Simplesat for Support: one-click surveys in the solved email, 9/10

Simplesat adds one-click CSAT, CES, five-star or NPS surveys to Zendesk's existing solved-ticket notification, so customers don't get an extra email. Surveys can have several questions with conditional logic, ratings and ticket data sync both ways, and the sidebar app shows feedback for the ticket and for the user. It has 28 reviews averaging 4.89 and was updated in January 2026.

Scores: reviews 1, price 2, works in the ticket 2, setup 2, maintained 2.

Caley Quinn wrote on its listing: "Our team was looking for a more customizable and personalized CSAT survey as the native Zendesk CSAT functionality is fairly basic and limited." Simplesat's pricing page lists Starter at $149 a month (300 responses), Pro at $299 (1,000), Premium at $599 (2,500) and Scale at $999 (5,000), about two months cheaper billed annually, with Enterprise through sales. Integrations are on every plan, and the 14-day trial runs at Premium level.

Best for: teams that want the survey inside the email customers already get.

Third tier, 8 of 10: CX Cards for Support

4. CX Cards for Support: survey answers that change the ticket, 8/10

CX Cards from VOC Metrics sends email or SMS surveys when a ticket is created, updated or solved, under conditions you set, such as high-priority tickets or tickets unresolved for a set time. Responses can update the ticket, raise its priority or trigger an escalation, and they're stored as Zendesk custom objects. Agents see the requester's answers in the sidebar. It was updated in April 2026 and has no reviews or recorded installs.

Scores: reviews 0, price 2, works in the ticket 2, setup 2, maintained 2.

Its pricing page lists Essentials at $125 a month and Pro at $199 for 10,000 email surveys, with Enterprise on enquiry. Read the table closely: the agent sidebar isn't in Essentials and costs $30 per agent per month on Pro, and the Essentials survey allowance is 250 in the plan card and 125 in the comparison table. The listing offers a 30-day trial. The score rewards what's published, not proof of use.

Best for: teams that want a bad rating to raise the ticket's priority without a person reading it first.

Fourth tier, 7 of 10: seven apps

5. Formcrafts: CSAT and NPS forms from a form builder, 7/10

Formcrafts is a form builder used mostly for ticket forms that create or update Zendesk tickets, and it also builds CSAT and NPS forms you send with Zendesk triggers, with ratings sent back into Zendesk. It has 22 reviews averaging 4.95 and an April 2026 update. Mathilde Hofshagen wrote on its listing: "Can't say enough good things about the Zendesk integration."

Scores: reviews 1, price 2, works in the ticket 1, setup 1, maintained 2.

It has no Marketplace install: the listing's button sends you to Formcrafts, and you map form fields to Zendesk fields in a workflow. The pricing page shows a free plan with 100 responses a month, then Standard at $39, Plus at $59 and Business at $129 a month.

Best for: teams that also want better ticket forms and can share one tool for both.

6. NPS and Survey: mixed question types in one survey, 7/10

GrowthDot's NPS and Survey combines NPS, five-star CSAT, yes/no and open-text questions in one survey, sent by Zendesk triggers. It asks for more detail after a low score, limits how often a customer is surveyed, and reports by agent, group or date. It has 4 reviews and was updated in September 2026. Ian Loizos wrote on its listing: "Support team extremely helpful, planned and implemented an upgrade within a few days."

The app sits in the nav bar and user sidebar, not the ticket sidebar, so it scores 1 on working in the ticket. GrowthDot's page prices it at $95 per company per month with unlimited surveys. The listing says the trial is 14 days; one section of the vendor page says 7.

Best for: teams that want NPS and CSAT in the same survey at one flat price.

7. Elvan: CSAT surveys with AI summaries, 7/10

Elvan sends CSAT surveys when tickets are solved or closed, and you design, schedule and analyze them in Elvan. The listing says it reads only the ticket ID, status and requester's name and email. The sidebar app is where you connect your account, not where results appear, so it scores 1 on working in the ticket. It has 1 review, fewer than five installs, and a May 2026 update.

Elvan's pricing page has a free Hobby plan with 100 responses a month and Pro at $49 a month for 1,000 responses, plus $0.01 per extra response. The page doesn't say which plans include Zendesk.

Best for: small teams that want a free start and a summary of written feedback.

8. SurveySparrow: a full survey platform with Zendesk rules, 7/10

SurveySparrow is a general survey platform with chat-style, NPS and classic surveys. The Zendesk app lives in the nav bar, where you create rules that email a survey when a ticket meets your conditions, with an optional delay of days. It has no reviews; the live listing shows an August 2026 update and 25+ installs.

Scores: reviews 0, price 2, works in the ticket 1, setup 2, maintained 2.

Its pricing page publishes a price for every plan but Enterprise, but Zendesk first appears in the Business plan's integrations, so the cheaper plans won't connect.

Best for: companies already running SurveySparrow for other surveys.

9. CSAT Viewer: Zendesk's own CSAT result next to the ticket, 7/10

Tripelhop's CSAT Viewer shows the result of Zendesk's customizable satisfaction survey in the ticket sidebar: the rating, the reasons chosen, the comment, and when the survey was offered and answered. It has no settings, stores nothing and changes nothing, which is why it scores 1 on working in the ticket. It was updated in October 2026 and has no reviews.

The listing shows $3 per agent per month or $39 a month for the whole account, after a 7-day trial. The two meet at 13 agents.

Best for: teams happy with Zendesk's survey who want agents to see the answer without opening reports.

10. CSAT Response Suite: ticket and requester CSAT plus a dashboard, 7/10

Gravity CX's CSAT Response Suite shows the current ticket's CSAT in the sidebar, plus the requester's recent feedback and a score from up to five earlier rated tickets. A nav-bar dashboard filters ratings by date, score, assignee, group and organization and exports to CSV, XLSX or PDF. It reads only and sends nothing to an outside server. It needs Support Professional or Suite Growth or higher, costs $1 per agent per month after a 7-day trial, and was updated in October 2026.

Best for: agents who should know a requester rated their last ticket badly before replying.

11. CSAT-Timer: CSAT on messaging tickets that close themselves, 7/10

Omni Wise's CSAT-Timer starts a sidebar timer when satisfaction is offered on a messaging channel and closes the ticket, tagged, if the customer doesn't answer in time. It can also create follow-up tickets. Per-channel rules use the Sunshine Conversations API on Suite Professional or above; a single master rule works on any plan. It costs $2.50 per agent per month after a 7-day trial.

Its last update was May 2025, between 12 and 24 months ago. For the native options on messaging, see custom CSAT surveys in Zendesk messaging.

Best for: messaging teams whose solved conversations sit open waiting for a rating.

Fifth tier, 6 of 10: two apps

12. CSAT.AI: AI QA with surveys attached, 6/10

CSAT.AI from Navedas is mostly a QA product: it scores conversations against your own rubric, adds coaching cards and clusters customer feedback, with "AI Surveys" that control delivery and survey fatigue. In Zendesk it's a nav-bar icon that opens the CSAT.AI dashboard, so it scores 1 on working in the ticket. It has 3 reviews and was updated in August 2026.

Its pricing page says Growth starts at $20 per agent per month billed annually, with a 21-day trial and Enterprise usually from $30,000 to $100,000 or more a year. A "starting at" price scores 1.

Best for: teams that want QA scoring first and surveys as part of it.

13. CSAT Opt-out: let customers stop the surveys, 6/10

DataFox Labs' CSAT Opt-out gives customers branded pages to opt in or out of CSAT surveys and lets admins manage those preferences from the nav bar. It costs $20.99 a month after a 3-day trial.

Scores: reviews 0, price 2, works in the ticket 1, setup 2, maintained 1.

The last update was April 2025, and it has no reviews or recorded installs.

Best for: B2B teams whose regular contacts are tired of a survey on every ticket.

Also listed, not ranked

Zonka Feedback, InMoment, Survicate and Qualtrics Task have marketing-only listings that link to the vendor, and Zonka's pricing page quotes per business. Zenloop Survey has fewer than five installs and no published price. SurveyMonkey's app sends to customer lists rather than after tickets, and its only review is more than six years old. Stylo's predicted CSAT scores and Klick-Zen's one-click rating buttons are scored in our other Zendesk roundups.

What survey apps change for an AI agent on the same tickets

A survey app adds events to tickets that an AI agent also works. On Zendesk, Macha's tools read the ticket, its comments and custom field values, and write status, priority, custom fields, tags, internal notes and public replies.

We check three things before an agent goes live. First, where the rating lands. Nicereply posts responses as comments and Survey writes them to ticket fields; a new comment on a solved ticket can match an agent's trigger and start a run nobody wanted. We use Macha's tag-and-exclude pattern so the agent's trigger skips tickets it has already handled, and we watch high-volume triggers for warnings. Second, which fields the app owns. We read the field definitions so the agent doesn't overwrite a survey field (see custom fields and AI agents). Third, what status the agent sets. Zendesk's survey and most of these apps fire on Solved, so if the agent solves tickets, its replies get rated too. That's useful data, but it's worth separating agent-solved tickets with a tag before you read the score.

Then we replay past tickets in a simulation and confirm whether replies post as internal notes or public before anything runs on its own.

Where AI fits

A survey tells you how the last reply landed. An AI agent writes the next one, sets fields and tags, and hands off what it shouldn't handle.

Macha runs AI agents inside the Zendesk account you already have, set up and monitored by the Macha team. It fits teams already running Zendesk who want agents acting inside the ticket on routine requests, with the survey app they already trust scoring the results. Pricing starts at $299/month for 750 tickets, with a trial of $50 of free usage (about 125 tickets), no credit card, no time limit.

Which one fits your situation?

  • You want CSAT, NPS and effort scores from one tool: Nicereply, from $59 a month billed annually.
  • You report in Explore and want answers in ticket fields: SweetHawk's Survey at $100 a month.
  • You want the survey inside the solved email: Simplesat for Support.
  • A bad rating should change the ticket on its own: CX Cards for Support, or Survey with a trigger on its field.
  • You're happy with Zendesk's survey and want agents to see results: CSAT Response Suite at $1 per agent, or CSAT Viewer.
  • Messaging tickets wait for ratings that never come: CSAT-Timer.
  • Customers complain about too many surveys: CSAT Opt-out, or a frequency limit in Survey or NPS and Survey.

FAQs

What is the best CSAT app for Zendesk?

On our rubric, Nicereply scores 10 of 10, with SweetHawk's Survey and Simplesat for Support on 9. All three write ratings back to the ticket and publish full prices.

Can Zendesk run NPS surveys without an app?

Zendesk's built-in survey is a CSAT survey with one rating question on Solved tickets. For NPS, Nicereply, Simplesat, Survey and NPS and Survey all offer a 0-10 question that sends from Zendesk.

How much do Zendesk survey apps cost?

On the apps we scored, the small per-agent apps run from $1 to $3 per agent a month, and CSAT.AI starts at $20 per agent billed annually. Flat-priced apps run from $20.99 to $300 a month, and plans priced by responses start at $49 a month (Elvan Pro), with free tiers from Formcrafts and Elvan.

Do survey apps work with AI agents in Zendesk?

Yes. Check where each app writes ratings, because a comment or field update on a solved ticket can wake an AI agent's trigger, and tag agent-solved tickets so you can read their scores separately.

How we researched this

On October 29, 2026 we queried the Zendesk Marketplace catalog, 1,812 apps, for the Surveys and Reviews category: 82 apps, 81 from third parties across 71 vendors, of which 9 have 10 or more reviews, 51 have none and 20 are marketing-only listings. We added apps from other categories that name CSAT, NPS or effort surveys.

We read all 13 listings on zendesk.co.uk the same day in a rendered browser, so the price panel and review text loaded, and read each vendor's pricing page where one exists. We used only plans the catalog marks as active. Last-update dates come from each live listing. We didn't install or test any app or contact any vendor, and one reviewer applied the scores to public pages only.
